
Before you apply you may want to try the self assessment form on the Home Connections website which will give you a rough idea of your priority points for housing if you choose to make an application to join the transfer register.
The first step is to complete an application form which you can obtain from the town hall or you can request one by contacting the freephone number. You can also download a form below:

We can help you fill out the forms either at your home or at our offices.
Once the forms have been completed we would like to meet you and discuss your re-housing options and prospects. If you prefer we can complete the forms and discuss them at the same time. Alternatively, you may wish to complete the forms and contact us when you are ready. We would like to tell you how to use the Home Connections website. We would be happy to do this at your home if you have internet access.
We would like to meet you but if you are happy to discuss your options over the ‘phone we can but we will not be able to give you a practical demonstration of the home connections website.
Completed forms should be sent to the Allocations team, Charles House, 375 Kensington High Street, London W14 8QH. You can also hand in completed forms to the TMO customer services advisor at the housing reception on the second floor at the town hall or your local estate office. Call us if you would like to know where to hand in your form.
We will register your application on the common housing register and for the Home Connections scheme as soon as possible but always within 10 working days of receiving your application. We will write to you to advise what bed size list you are registered on, your position on the list and how your points were calculated. We will also advise you of other housing options such as moving out of London and mutual exchanges which you may wish to consider. Further details are on the Home Connections website.
